Peritoneum vaginoplasty (PPV) for Gender Affirming Surgery

Information on Peritoneal vaginoplasty

The peritoneum is a sheet of smooth tissue that lines your abdominopelvic cavity.

Peritoneal vaginoplasty is considered an alternative approach to vaginoplasty compared to using the penile skin or colon tissue. Each method has its own set of advantages and considerations, and the choice of technique often depends on factors such as the surgeon’s expertise and the patient’s preferences.

Peritoneal vaginoplasty can serve patient’s preferences who would like to get a better elastic lining and moisture inside the vagina than the skin graft technique.

Dr. Chettawut and his MIS surgeon partner have the expertise and experience to perform a successful peritoneal vaginoplasty using a laparoscopic technique to harvest a portion of the parietal peritoneum at the posterior abdominal wall. Peritoneal dissection was done to create a peritoneal flap 12×20 cm feed by mesenteric vessels, which can be formed in a 12-15 cm depth tube. The peritoneal tube was brought out through the neovaginal tunnel.

Our surgical technique is superior to the traditional Davydov Procedure as the harvested peritoneal tube of 12-15 cm is adequate to be a total lining of the vagina (without the combination of inverted penile skin or skin graft)

Laparoscopic peritoneal vaginoplasty is considered a minimally invasive approach, which may result in smaller incisions, minimal pain, and potentially quicker recovery times compared to traditional open surgery.

The peritoneal vaginoplasty procedure can be performed in primary gender reassignment surgery or corrective vaginoplasty, called secondary peritoneal vaginoplasty.
